MASSAGE REQUEST FORM FOR NEW JERSEY

Hi, I’ve just started my practice in NJ. I have a space at Align Wellness in Milburn NJ and am taking appts one by one on Thursdays and Saturdays. My introductory rates are $145 for one hour and $190 for one and a half hour. A deposit will be needed since most clients are new. I will always have a 24hr cancellation policy.
